Remote monitoring programs help patients and providers effectively manage chronic conditions and incorporate health into patients’ day-to-day lives. Offered in a number of primary care practices within the Partners HealthCare network, remote monitoring programs allow patients to track their health, securely share their health data with their provider and become collaborative partners in their care. The remotely monitored data – including blood pressure, blood glucose and weight – is viewable within the medical record, giving clinicians a more complete picture of the patient's condition in order to more effectively assess the patient's status and provide just-in-time care. Patients can also monitor their own health data via the Partners secure patient portal.
Partners HealthCare was one of the first large medical delivery systems in the US to seamlessly incorporate home monitored vital signs with the patient medical record, using our proprietary technology platform. This initiative is helping make patients more accountable for their health and gives patients the tools and information needed for self-management.
As the technologies to remotely measure, securely communicate and effectively present vital signs improve, we are focusing efforts on improving cost-benefit and operational efficiencies of remote monitoring, and testing new, automated feedback tools to optimize patient engagement.
